FORM DISCLOSURE AND SUBMISSION

Please Read the Following Carefully. After submitting this form, you will be redirected to a page to pay your Rental amount due.

NOTE: Boone Woods is located in Burlington off Burlington Pike, on Veterans Way, in back of the YMCA and Huntington Bank. Event will be held in the back of the park at Shelter #3. There will be signs to lead you to where the event will be held. You may drive to your booth space to unload. Parking is limited so you will be asked to park in an adjacent lot.

SPECIAL NOTE: The Friends of the Shelter Committee reserves the right to exclude any vendor who does not adhere to the PetFest 2019 Rules and Regulations, or which the Committee finds inappropriate to the spirit of the Fest.

UNLESS NOTIFIED YOU MAY ASSUME THAT YOUR APPLICATION HAS BEEN ACCEPTED. DUE TO TIME LIMITATIONS, CONFIRMATIONS WILL NOT BE SENT. YOU MAY CONFIRM RECEIPT BY CONTACTING [email protected]

You will receive your space/booth placement from the Friends of the Shelter Committee member at the registration table on June 28th, after 9:30am. Event officially starts at 11:00 am and ends at 4:00 pm.

ANY VIOLATION OF THE RULES AND REGULATIONS PRESENTED BY THE FRIENDS OF THE SHELTER COMMITTEE WILL RESULT IN CLOSURE OF YOUR BOOTH.

The state of Kentucky requires special permits for any gambling, lottery or raffle.
